ViluaCARE - Using the example of patient support programs

Program Kick-Off

The person insured receives full support.

  • The physician informs the patient about an existing PSP.
  • Alternatively, the patient is informed via other sources, e.g. his own research or support groups.
  • After the registration is completed, the personal coach can be contacted, either via phone or digitally on our communication platform, the Health Cockpit.
  • The Health Cockpit acts as the central control system for all activities and communication of everyone involved.

  • The individual coaching is supported by digital options.
  • Certified prevention coaches also possess a medical qualification and they are well trained by the client beforehand.
  • Together with the participating patients, personal behavioral patterns are specified and regularly updated for the achievement of individual health-related goals.
  • Topics such as nutrition, sports, stress as well as information regarding interactions between medications are further components.
